Create and Update Your CAQH Profile in Miami and South Florida

It is a shared online database where a provider enters their professional information once and authorizes health plans to retrieve it for credentialing. Plans rely on it to avoid collecting the same documents separately.

We register you in CAQH ProView or recover your existing CAQH ID, enter your education, training, work history, licenses and practice locations without gaps, upload the supporting documents, authorize the plans that need access and complete the initial attestation. The finished profile is checked against your NPI record and Florida license so the three match.

Your Florida license, DEA certificate if you prescribe, the face sheet of your professional liability policy, a current CV, a W-9 for the billing entity and board or specialty certificates. We collect them once and keep copies for future updates.

Periodically CAQH asks you to confirm that the information in the profile is still accurate. If the attestation lapses or a document such as your license or insurance expires in the profile, plans see it as incomplete, so we track those dates for you.
